Clinical Clues in FUO
- Conduct a thorough history, including a review of systems and detailed physical exam, including rectal/pelvic.
- Follow up on clinical clues, such as cough, diarrhea, or abdominal pain, with targeted investigations.
Baseline FUO Labs
- If the history and physical exam are inconclusive, conduct baseline labs: CBC, CMP, COVID, HIV, monospot, ANA.
- Also test for multiple myeloma, TB (PPD or QuantiFERON), urinalysis, chest x-ray, blood cultures, and consider echocardiography.
Imaging in FUO
- When initial tests are negative, a CT scan of the chest, abdomen, and pelvis is appropriate.
- Consider PET/CT as it's more sensitive for detecting tumors and infections due to their high metabolic rate.
