I'm not one to run with fantastical headlines, but the truth is that we are facing an infertility epidemic. And, this is all heartbreaking for the people dealing with infertility, and for us as a society. Today, I'm honored to be joined by Dr. Shanna Swan on The Egg Whisperer Show to talk about this upsetting truth. Dr. Swan is one of the world's leading environmental and reproductive epidemiologists and a Professor of environmental medicine and public health at the Icahn School of Medicine at Mount Sinai in New York City. An award-winning scientist, her work examines the impact of environmental exposures including chemicals such as phthalates and Bisphenol A, on men's and women's reproductive health and the neurodevelopment of children. She just recently published her book, "Countdown: How Our Modern World Is Threatening Sperm Counts, Altering Male and Female Reproductive Development, and Imperiling the Future of the Human Race." We will be talking about the data and information that Dr. Swan has gathered, the environmental factors that are playing a role, and what we can do to reverse some of the impact. On a recent episode of Ask The Egg Whisperer, Mary sent in a question. She's 33 and has been trying to conceive for two years. Here's what she wrote, "I've been trying for our first baby for the past two years. I had a miscarriage this past summer, and I'm now seeing a fertility doc to see if IVF is the right choice for me. The blood work came out pretty much fine. Tubes are open, and my uterus looks good. So I'm not sure why we're not able to conceive naturally. My husband and I have also gone through genetic testing. I carry a gene for thalassemia. Do you suggest I go for IVF or try other methods first? And what all questions should I be asking my REI before deciding on IVF?" On a recent episode of Ask The Egg Whisperer, Ana Lucía shared a great question. Here's what she wrote, "Thank you Dr Aimee for your show! I just had a miscarriage. I am 42, my progesterone and estradiol levels are very low (8 and 147). I've been taking estradiol 6mg and Crinone daily. Why are they low and how can I improve them? Thank you very much for your guidance." In answering this question, it's important to understand how you're taking progesterone (Crinone) and estradiol. Let's break this down a little: for progesterone, the level in your blood is based on the mode of administration. When I have a patient on Crinone (vaginal form), and their progesterone level is 8 - that's because you're placing it vaginally. It has to go through the walls of the vagina, get absorbed, and then go into your bloodstream. You could add more progesterone to your protocol to raise your progesterone, but I certainly don't want you to worry that your progesterone levels were too low for a healthy implantation to occur if this is a level checked after a frozen embryo transfer. I like the level to be around 10 in a medicated FET cycle if using vaginal progesterone. These numbers sometimes make us worry unnecessarily (especially when we go to Google for answers). You might see things that make you really upset, like information saying that your progesterone should be over 20. And, that's sound advice if you're using an injectable form of progesterone. But, with Crione, which is a vaginal form, you're just fine. Talk to your doctor and share your concerns for additional reassurance. In this episode, I'm joined by Dr. Geoffrey Sher of Sher Fertility Solutions. He is an internationally renowned fertility expert. He trained under "The Fathers of IVF, " Drs. Patrick Steptoe and Robert Edwards in the UK. In 1982 he established the 1st private (non-university based) IVF program in the US. Over a career spanning almost 40 years Dr. Sher has been influential in the births of over 17000 IVF babies and has helped fashion the entire field of ART. He is the author of " In Vitro Fertilization, the A.R.T. of Making Babies" and (more recently), "Unexplained IVF failure and Recurrent pregnancy loss: The Immunologic Link"." Today he's joining me to talk about Immunologic Implantation Dysfunction (IID), which may be a factor in unexplained IVF failure and recurrent pregnancy loss (RPL). He will be talking about how the endometrial lining plays an important role in embryo implantation, because as he shares, "you can't put a good seed in bad soil, or a bad seed in good soil." The time to talk about "everything I can possibly do to give myself the best chance of pregnancy success" is BEFORE your embryo transfer. Not after. This is why I discuss tests like the ERA test and Receptivadx test with all my patients before transfer. Every embryo is important to me and I want to make sure I'm doing the very best job for every patient I meet.⠀⁠ ⠀⁠ I also know that there's a virtual alphabet soup of three letter abbreviations that surround the whole fertility journey, so this week on the podcast, I'm breaking down the steps that I take when doing the ERA (Endometrial Receptivity Analysis) test, and how it could help you. The ERA tells us exactly when to start progesterone before a transfer to give the embryo the highest chance of implantation success. Think of it as a personal evite to your Embryo Transfer Party! In a way, the test tells you exactly when to show up for the party. IVF is a very emotional journey. It can be riddled with fear, doubt, and is full of periods of waiting and wondering. For my patients, I want to reduce any emotional or physical burdens by creating an IVF checklist for them. It covers the core essentials of everything we need to do, so there are no conversations that begin with, "I wish I would have…" down the road. I don't want you to have to wonder if you've done everything you can to have a successful IVF outcome. This is true if you're my patient, or you live half-way across the world. I want to equip you with a complete IVF checklist. I want you to feel prepared going into IVF so there is less chance of having any regrets if things don't pan out as you hope they will. Kim recently wrote in to ask the Egg Whisperer this question, "Hi Dr. Aimee. I am 34 years old. Recently I was diagnosed as PCOS due to the number of follicles on my ovaries and my acne problem. Before I began my IVF journey, I was diagnosed with IBS (possible Celiac disease or gluten sensitivity that had yet to be confirmed), hypoglycemic, and severe vitamin D deficiency (13L). My question is, is there any correlation between IBS or Celiac disease, Vitamin D deficiency and hypoglycemic (fyi my BMI is 24 and I am not diabetic) and infertility? If so, should I postpone my FET on 04/30/21? What can I do to give myself a better chance of having a successful IVF if I do decide to continue with the FET?" I share about the known relationship between IBS and PCOS, and information about some more recent studies about Vitamin D deficiencies. You'll want to tune in for the information.
