#5: The Road to Hell is Paved with Percocet
In this episode, Tim and May discuss the single biggest mistake in the history of Medicine. The overprescribing of opioids started with a five sentence letter to the New England Journal of Medicine in 1980. This brief letter was quoted 608 times in Medical literature, espousing the safety of opiate prescribing. We dive into the history of opiates, opiate prescribing and drug abuse in general. We talk about how a simple misinterpretation of data caused the medical establishment, big pharma and government regulators to get it wrong and kill hundreds of thousands. This peer pressure and group think produced three big lies: 1: We could do it better with opioids “this time’ compared to a hundred years ago 2: Medicalization of opioids makes it “okay” 3: Treating acute and chronic pain has always been a challenge, therefore we have to make a medical “solution”. And we also have a good laugh at the past and near present advertisements. #3: Medicine... Was It All Worth It?
When you think about going into medicine, do you think about why you chose the field? Do you look back and wonder, “I thought medicine would be so much different than I know it to be now?” To choose a subspecialty today, you have to have an immense amount of confidence in the unknown. Tim talks about how this can become an undercurrent of self-doubt. #2: Basement Blowups to Urgent Care Docs
Looking back on our medical journey, Tim and I reflect on what brought us to Medicine. Tim comes from a medical family where his father was an independent GP, doing OB/anesthesia/call/everything. He understood pretty early on that medicine wouldn’t be an easy path. I got the science bug early on but didn’t come from a family of doctors, or anyone with a college education. The pressure was real, though. I remember blowing up stuff in my parent’s basement and loving science in general. Plus, I just really enjoyed medicine on TV and so it kind of became my label. The needle was sunk into my family’s viens more than mine: send me straight to university into pre-med. This episode is personal. It’s honest. #1: Should you sink the needle?
You knew we would kickoff with Covid right?! Due to the timing of the pandemic and requests from friends, we knew we had to talk about COVID-19 vaccines. We are keeping it simple and talking about normal stuff like common misconceptions, how the vaccine works and what are their side effects? Who do we think should get the shot? And, who should not. What we know right now is that there are 5 types of vaccines that made it into phase 3 trials but 3 are really important to pay attention to.
