#24: Drugs In my Pocket (and I don't’ know what to do with then)
This week we are talking about “drugs”, specifically the world of PBMs with Dr. Lee Strandberg. Pharmacy Benefit Managers(PBM) are the middlemen between your pharmacist and your prescription and your insurance company. Yep.. more middlemen. If you have never heard of the term PBM, well you are not alone. Most people have no clue that they exist, how they work and how the cost or medications is controlled, negotiated or paid for. They are the one paying the pharmacist for your medication! While there are approximately 60 PBS, the 3 largest ones currently control 80% of the market. We discuss both arms of the pharmacy world- how the pharmacy acquires the medications and purchases them. Hang on as we unravel the mysterious chain of events on how the pharmacy gets paid by the PBM and how they negotiate the prices with your insurance company. We do get a little deep in the weeds on cost control and price negotiations, so pay attention. We only WISH that getting a Zpack could be as simple as buying a head of lettuce from your local farmers market. Dr. Lee Strandberg is our guest this week who intimately knows the ins and outs of the complex layers of the pharmacy world. He is a Professor Emeritus, Pharmacy Economics and Public Health Oregon State University and was Director, Managed Care pharmacy for SHS Plans from 2005-2011. He was appointed by Oregon Governor Kitzhaber to the OR Health Policy Board, Medicare-Medicaid Integration of Care and Services work Group which wrote part of the CCO and reporting structure. He was a committee member on the OR Evidence Based Review of Prescription Drugs Preferred Drug List. During the past 40 years he has presented papers, seminars and consulted on the impact of pharmaceutical use on health care costs and quality. #22: “When the Oncologist gets Cancer” The Doctor as Patient with Dr. Reema Batra

Dr. Batra is a Board Certified Medical Oncologist and Hematologist. #19: Sinking the Covid Needle or Not? (Part 2 with Dr. Peter McCullough)
Hang on to your seats, this is a long one! This week is a continuation from our podcast last week with Dr. Peter McCullough regarding Covid-19! Last week we discussed evidence based treatments for Covid-19 , best practices to avoid hospitalizations and death plus what resources we have. This week is all about the vaccines, safety, adverse events and RISK!. Plus is there a bigger theme in all of this? It feels like we are fighting a war not against Covid, but against each other. With opposing sides in the public and medicine, to vaccinate or not to vaccinate; who , what, when and why IS the question! We used to think asymptomatic spread was frequent and now we are learning otherwise. New evidence from published data available from tracking over 10 million people in China shows there is a less than 1% spread from asymptomatic people. The evidence seems to show we need to “Stop” testing asymptomatic well people! What we are doing in public health now seems wasteful, expensive. Why are we now still having to wear masks if we are vaccinated if it’s fully safe and effective as promoted? Next we look at extreme differences between the states handling the pandemic response from Michigan to Texas. Taking it further, look up north to the border to home and native land in Canada. The border is closed, Covid hotels are mandatory for noncitizens entering for ANY reason, with no regard to their health, immunity or current vaccination status. Draconian or Totalitarianism? You won’t believe it, but it’s TRUE! Johns Hopkins published a booklet in 2017 referencing THE SPARS PROJECT. It was a “response” document and plan to deal with the next pandemic , predicting even the exact genus and species that would hit the world in the near future. You can read how they had already mapped out how to handle, shut down and analyze a global crisis. Next we get into how the response to Covid-19 feels to be weaponized. Schools, businesses, churches, and countries shut down and often with massive fines for not adhering to authority. Is this really The GREAT RESET? We move on to discuss how the vaccine initially was promoted as a “volunteer” basis to protect those at risk. Now we are seeing the implementation of financial coercement, Universities mandating it, talk of vaccine passports, business threatening to fire employees who are unvaccinated. What!! While Dr. McCullough is pro- vaccine, he definitely believes that Covid-19 vaccination may not be appropriate for everybody. Covid survivors, potential childbearing women and pregnant women were ALL excluded from the trials, why would we expect them to blindly get the vaccine without any data? This is a risk too big to ask for many people and one that is naively endorsed by many. The CDC is very clear on their statement regarding who should get the vaccine : ask your doctor if the vaccine is right for you. They DON'T GET TO DECIDE! It is a discussion and decision between the physician and patient. No other vaccine( flu, pneumococcal, shingles, etc) has been pushed this hard to the masses in the history of vaccines. To date there are 3514 deaths associated with the vaccine: anaphylaxis , reactogenic deaths (from blood clots, high fevers, etc) and then thrombotic events that occur farther out in time. The numbers for vaccine related adverse events is over 40,000 per 100 million. Something to consider: Is Johnson and Johnson the safest vaccine because it is only ONE vaccine? Finally: The proposition of vaccinating healthy low risk people to protect the population at large is fraught with moral implications. There WILL be college kids who die due to the rare event from vaccine induced causes .. from being coerced into the vaccine because of pressure from the masses. How do we assess risk, benefit and sort through the data………….. What an honor to have this intelligent discussion and input from a world renowned and highly published physician. Dr. McCullough is a practicing internist and cardiologist and Professor of Medicine at Texas A & M College of Medicine. He is board certified in internal medicine, cardiology, and clinical lipidology. He also has a Master of Public Health from University of Michigan School of Public Health. Dr. McCullough has broadly published on a range of topics in medicine with > 1000 publications and > 600 citations in the National Library of Medicine. #17: The Doctor as Patient with Dr. Manisha Rayavarapu
Today’s episode is the first in our series on “The Doctor as Patient”. So many times at work I have been shocked when patients find out I had the flu, had surgery or experienced a “real medical” problem. No we aren’t superheroes, and we DO share your pain. When the doc becomes the patient, they often will experience the same struggles ,vulnerabilities and fears. This can be a very eye opening experience and impact how we see and treat our patients. This week we have the privilege to hear from Dr. Manisha Rayavarapu. She is a Family Medicine physician, diagnosed with Lupus during her 2nd year of residency training. She describes how her faith along with her husband, have been the most important support on her journey. She shares her struggles with navigating the health system ,uncompassionate colleagues and administration, to family who may not understand how her illness affects her life. She has gone on to write a book on her experiences, struggles, fears and strength on coping with her ‘new normal’. Her memoir, “Butterflies, Boards and Blessings” is her way of helping educate and raise awareness about lupus. #16: Doctales with Cocktails: Doctor Heal Thyself
Yippee it’s time for another Doctales with Cocktails! Cheers AND…..today we are diving into the beginning of our series on the “Doctor as Patient”. We love to poke fun at ourselves, so we are sharing a few ‘memoirs” from our morbid past. Did you know Tim tried to be stoic beyond his common sense? Yep and he managed to land up with extended “staycation” at the hotel hospital, experiencing the pleasures of baseball and PCA pump full of morphine. We share our first childbirth experience. Come on who are we kidding- we all know it was actually MAY’s marathon experience! Tim’s participation in the event was mostly a few minutes of “fun” and then having to split his brain between playing doctor or the dad card.
