Testicular Sperm May Improve Pregnancy Rates, a Discussion with Dr. Blake Evans

Recently, Suzanne wrote in to ask if working a night shift may negatively impact her chances of becoming pregnant via embryo transfer. Here's what she has to say, "I'm wondering if you think that it's very bad to work night shifts when trying to get pregnant with an embryo transfer. I also work some days and evenings, but mostly nights. And I've heard different things about it. If it's good or not, what do you think about it? I have type 2 diabetes, PCOS, and sleep apnea. Can I be successful with fertility treatment? (Ask The Egg Whisperer)

Recently, Yamilka sent a question in to "Ask The Egg Whisperer," and here's what she said: "I am from Florida, 36 years old. I'd like to start the fertility process on my own. However, I'm overweight have type two diabetes and sleep apnea. I was diagnosed with PCOS. For the last couple of years, my doctors weren't clear if I have it or not. I have regular cycles that lasts around 32 days and I want to do IUI or IVF soon. What are the possibilities of me receiving treatment successfully?" My short answer is that Yamilka's chances of having a healthy pregnancy at age 36 are really, really good. While she does have PCOS, I would want to see what we can do to help you be even healthier. One thing that has helped patients of mine with similar situations is seeing a doctor, like Stephanie Fein, who specializes in fertility, and helping people reach their body size goals. What should my progesterone level be before and after an frozen embryo transfer? (Ask The Egg Whisperer)

Recently, a listener sent a question in to Ask the Egg Whisperer. Here's what she asked, "Hi, Dr. Aimee. I recently had an FET(Frozen Embryo Transfer) that ended in a miscarriage at six and a half weeks with a high grade, PGS tested embryo. I did the era test. We did the transfer on the right day. I was wondering what should my progesterone level be before and after the frozen embryo transfer?" My short answer is this: before the frozen embryo transfer, on the lining check day, you want it to be less than one. I like it to be around 0.5. On the transfer date, I don't check progesterone level, but if there's a concern that someone's having a reaction to the progesterone, I check it. I like it to be at least 20, if you're doing intramuscular progesterone. The next week, with a pregnancy test, I always check progesterone with my HCG levels. I like the progesterone level to be over 20, ideally around 30. Tune in to hear the full answer to this question and many others. Can the ERA test be done to figure out the best implantation window during a natural FET cycle? (Ask The Egg Whisperer)

Recently, a listener sent this question in to Ask The Egg Whisperer: "Hi Dr. Aimee! Thank you for all the work you're doing and for giving us the opportunity to ask questions. I listened to your last show on the ERA test. And I would like to know whether an ERA (Endometrial Receptivity Analysis) test can be done to figure out the best implantation window in the case of a natural FET (Frozen Embryo Transfer) cycle?" I always love answering these questions. Let's look at the basics of IVF before we talk about the ERA test. I like to think of it in three stages: 1. Egg and Sperm come together to make an embryo 2. Embryo transfer preparation 3. Embryo transfer The answer is: Yes, I do the ERA test in natural cycle transfers as well, because I want to get it right. I want to know exactly when a patient should do their trigger shot. I want to know exactly what time I should be doing that transfer, so that I never look back and say, "Oh, I wonder if the reason why this test is negative is because I didn't get the timing right."
