I am 37 and have endometriosis. What should I do to prepare for my FET? (Ask The Egg Whisperer)
Lindsay recently wrote in to Ask The Egg Whisperer. She is 37. Here's what she wrote, "I have a diagnosis of endometriosis. I have endometriomas on my ovaries, but have not been diagnosed surgically. I've been trying to conceive for two years. First on a natural program and now an IVF. I've had three egg retrievals, one canceled cycle because they didn't respond to medications and had only two follow-up calls and they resulted in two PGS euploid embryos total one day six and one day seven. I'm now preparing for my FET December 18. Do you have any suggestions to increase my chances of success. We want two children, and this is our only transfer. We are considering transferring both embryos, but haven't decided yet. These numbers are so precious to us since it took us so long to get here, COVID delays and all, and I would really appreciate your second opinion." My partner had a rapid decrease in sperm. What tests or supplements should we consider? (Ask The Egg Whisperer)
Recently, Corey sent a question to Ask The Egg Whisperer. Here's what she had to say, "My partner's most recent semen analysis was not great at all. As a quick background, January, 2020. Twenty-five million, with fairly low motility. July 2020, 28 million with an even lower motility rate. In the August and October analysis, they couldn't find anything. He took pregnyl shots for three months before our last test, also taking a few supplements daily. Do you have any recommendations on supplements that might increase our chances or testing that we should be doing to see why this was such a huge drop in sperm?" I have horrible pain at ovulation, and a canceled IVF. What tests should I have done? (Ask The Egg Whisperer)
Kate recently wrote in to Ask The Egg Whisperer a question. She is 32, and she has a lot of pain during ovulation. She has one failed round of IVF due to being a poor responder. Here's what she had to say, "Roughly every month, I am in horrible pain around ovulation. Sometimes it hurts so bad to even stand up. It's only on my left side. I also get extremely bloated and uncomfortable in the lower abdominal area. Neither my GYN or fertility specialist have an idea of why this is. Do you have any suggestions of what it can be? What tests could I have done? I had an HSG with suspected left fallopian tube blockage, and a sailing sonogram that showed a beautiful uterus. I'm 32. My AMH is 0.7. FSH is 7, and Estradiol is 64. I'm 40, and starting IVF. How do I make the most of it? (Ask the Egg Whisperer)
Recently, Jamie wrote in to Ask The Egg Whisperer a question. Here's what she had to say, "Hi Dr. Aimee! I just turned 40 in October and had a natural pregnancy that ended in miscarriage in November. I am starting IVF to get pregnant again more quickly due to my age and the length of time it usually takes me to get pregnant. I have had 2 miscarriages and 1 beautiful daughter. My question is related to timing of cycles and transfer. Is it worth focusing on completing more than 1 cycle (say up to 3) if I only get 1 normal embryo from my first cycle? And then, focus on transfers with what I have? I am worried about transfers taking time due to failed transfers and/or miscarriage and losing more ovarian reserve due to time lost. Why I Throw Egg Freezing Parties
I started throwing Egg Freezing parties as a fun way for people to learn about fertility. There is humor. There is honest talk. Mostly, I throw them because I want every person to be empowered with knowledge about their fertility. It's really as simple as that. Egg Freezing parties are safe and surgery-free gatherings of women (and sometimes men) to learn more about the process of freezing your eggs (or sperm). Beyond that, we talk in general about fertility and how every woman (and man) can take a proactive role in preserving theirs. I started Egg Freezing parties for a simple reason. Older women were in my office struggling with fertility only to wish they had known more when they were younger. Had someone told them to get their fertility levels checked and to consider the option of preserving their eggs they most certainly would have. I don't want to hear years from now that women wished they'd known more about their fertility. This is what drives me to throw parties to make sure everyone knows they have options! PGT-A with guest Dr. Eric Forman
Dr. Eric Forman is the medical and lab director at Columbia University Fertility Center where he oversees the medical practice as well as IVF embryology and andrology labs, while also actively seeing patients. He set the standard for eSETs (elective single embryo transfers) across the globe. And, he revolutionized how we freeze eggs, with his research proving that the rapid vitrification technology does not increase the risk of genetic abnormalities in embryos. Dr. Forman is joining me today on the Egg Whisperer Show podcast to discuss a hot topic in fertility medicine: PGT-A. Fertility Expert Series: Eric Forman on PGT-A
PGT-A is a hot topic in fertility medicine and I'm so happy that Dr. Eric Forman joined me to talk to us about a recent Chinese study published in The New England Journal of Medicine. Takeaways: - this study is getting a lot of attention because of the prestigious journal it's published in - the study design isn't how we do things in the real world: we don't just biopsy three embryos - the average age was 29 of the patients and mean egg number 20 and we know from previous studies that PGT-A as a selection tool may help women over age 37 more than younger women - the cumulative live birth rate was higher in the PGT arm - the study provided reassurances about PGT-A: the biopsy is safe, the group who did it had a higher implantation rate, similar cumulative live birth rate with fewer transfers, and fewer miscarriages. There were more congenital abnormalities in the non-biopsy group. A big thank you to Dr. Forman for breaking this all down for us and for sharing how PGT-A can improve the live birth rate on the first transfer and make elective single embryo transfer the standard of care across ages. I'm 41 and a carrier for a genetic disorder. Should I do another round of IVF now, or wait until after a healthy pregnancy with our one good embryo? (Ask the Egg Whisperer)
Recently, Elizabeth shared a question on AskTheEggWhisperer.com. She is 41, and she and her husband are carriers of a recessive disorder that results in severe physical and mental disability. Here's what she says, "I'm 41 AMH 0.95. We had a little girl naturally when I was. 40, but she didn't survive. We have a 25% chance of recurrence to screen for the disorder. We did our first round of IVF and we got results back today out of seven eggs, five fertilized, three made it to blast. We have one normal that is not affected. There was another normal blast, but it was affected, and there was one with abnormal chromosomes. We're so happy to have a good one to use. We might want two children though. I have two more rounds covered by insurance, but they can't be used for embryo banking. Should I wait until after a healthy pregnancy with our good one to do IVF again? I have hypothalamic amenorrhea, and I've had two egg retrievals, with poor fertilization. What do you recommend? (Ask the Egg Whisperer)
Recently, Tracy shared a question on AskTheEggWhisperer.com. She is 29, and her husband is 39. Here's what she says, "I have hypothalamic amenorrhea. I've had two egg retrievals. The first egg retrieval, three fertilized after seven retrieved, one blast tested abnormally. The second retrieval, we added an Omnitrope and dexamethazone during stimulation. ICSI and calcium solution to the eggs. Five retrieved, one fertilize, one blast, and it was normal. I eat very well, I take supplements, I meditate, and I am doing acupuncture. Any ideas on why have such poor fertilization? Do you think I have poor quality eggs, or is that the protocol that's hurting them? Should we be testing my husband's sperm further?
