Tihana Bicanic, an infectious diseases clinician and mycology expert, leads a deep dive on rezafungin with trial updates and practical OPAT experience. They cover weekly dosing and PK/PD, resistance concerns during the long drug tail, ICU and OPAT logistics, off-license uses for difficult infections, and cost versus hospital discharge tradeoffs.

Tihana Bicanic, infectious diseases clinician and medical mycology expert, joins to unpack echinocandins. They cover what echinocandins are and how they work. Short talks on clinical impact for candidemia, limited activity against other fungi, dosing differences, pharmacokinetics, resistance via FKS mutations, species-specific susceptibility, side effects, and stewardship approaches.

Anastasia “Tash” Theodosiou, a clinical microbiologist researching the microbiome, and Natalie Bennett, a Green peer and consumer-products regulator, discuss consumer biocides. They explain what biocides are, regulatory confusion, marketing claims like “kills 99.9%”, microbiome harms and resistance pathways. They also cover real-world examples, the proposed Biocides Bill and why some treated products may fail necessity tests.

They break down azole classes and when to use topical versus systemic agents. The conversation covers mechanisms of action and why azoles are fungistatic for Candida but fungicidal for molds. Fionnuala Cox, a consultant immunologist specializing in drug allergies, joins the discussion to illuminate the complexities of cephalosporin allergy. The conversation centers around the innovative CEPH-FAST risk assessment tool, designed for accurate allergy identification. Fionnuala explains the importance of delabelling antibiotic allergies, the challenges of skin testing, and the significance of understanding hypersensitivity reactions. They also delve into the practical applications of CEFAST, cross-reactivity guidance, and the implications of retaining allergy labels in clinical settings.
