New, modern antibody–drug conjugates (ADCs) have emerged as promising additions to the treatment arsenal for patients with lung cancer, with the first HER2-targeting ADC recently gaining FDA approval for patients with unresectable or metastatic NSCLC whose tumors have HER2 mutations. Clinical trials are also underway with novel ADCs targeting HER3, TROP2, CEACAM5, and MET, among others, and have been yielding encouraging results to date. What are the unique structural and mechanistic features as well as efficacy and safety profiles of these ADCs? Where are they likely to best fit in the NSCLC treatment algorithms? What practical considerations should be taken into account when integrating these therapies into treatment plans?
