Triage Based on Clinical Needs
- Admit children with pneumonia if they need oxygen, IV fluids, or have significant risk of deterioration.
- Consider family access to care and ability to complete antibiotics when deciding outpatient vs inpatient management.
Outpatient Pneumonia: Antibiotics Recommended
- Most children with pneumonia who are well enough to go home should receive antibiotics targeting bacterial pneumonia.
- Amoxicillin is preferred due to excellent coverage of Streptococcus pneumoniae and low resistance rates.
Choose Antibiotics Wisely
- Avoid azithromycin for typical pediatric pneumonia due to high resistance in Streptococcus pneumoniae.
- Avoid oral cephalosporins as first-line since they have poorer pneumococcal coverage compared to amoxicillin.
