if there is a lingering question it is related to this sentence;

How is it possible for the questions and requests to change from one day to another simply as our understanding changes?
Say a therapist….. just because their understanding shifts, there will still be clients to be seen who had made their appointments prior to the shift in understanding (of the therapist) Same client, same concern, what is different as a result of a shift in understanding?
Is it that the way the concern is heard/addressed by the therapist that changes due to the shift in understanding?
If so, fantastic, I get it.
If not, I have no idea what you are talking about, and please help!
