In the last two decades, there have been very limited advancements in the treatment of HR+, HER2- early breast cancer, and a particularly marked unmet need has existed for patients at the highest risk of recurrence. Fortunately, there is new hope of improved outcomes for these patients following the recent FDA approval of the first CDK4 and 6 inhibitor for the adjuvant treatment of patients with HR+, HER2-, node+ early breast cancer at high risk of recurrence and a Ki-67 score ≥20%. Shortly thereafter, guideline updates were released that recommended adjuvant CDK4 and 6 inhibitor therapy combined with endocrine therapy to a broader population of patients at high risk of recurrence. The emergence of this new therapeutic option along with varied indications and recommendations has resulted in much uncertainty about which patients should and should not receive adjuvant CDK4 and 6 inhibitor therapy, and how to best integrate it into clinical practice to meaningfully impact outcomes in this challenging patient population. In this educational activity, two leading experts deliver pertinent updates and use patient cases to provide practical guidance for the multidisciplinary team on how to navigate the changing standards of care in HR+, HER2- early breast cancer.
