The location where the distal biceps tendon turns most sharply towards the anticoebital fossa is marked. The distance between the crease and the cusp is the BCI. One may also note loss of more supination than flexion strength in lateral epicon delitis. There will be pain with resisted forearm pronation and wrist flexion, but no instability or apprehension with a valgus stress or a milking maneuver. So this is used to differentiate from MCL injuries.
