On page 31 of the book, you talk a little bit about sources of defects and rework. It says requirements and design are implicated in a large percentage of bugs with some studies showing them responsible for more than 50% of all defects. You seem to be implying here that you can have bugs and defects in your requirements and in your design. That one, I feel pretty confident. Most of those issues, especially the ones that cause a large amount of rework were actually introduced all the way back in the requirements.
