Atrophied or fasciculations tend to be seen more in lower motor neuron lesions, although long-standing upper motor neuron lesions can cause some atrophy. Unilateral weakness is an operational lesion until proven otherwise. bilateral leg weakness is a spinal lesion to proven otherwise.
