The Impact of Height and Width on Accessibility

The browser doesn't actually know what size the image is until it's been loaded. It used to be the case that people set the width and height of their images so that there was space for served for the image as well. But now because of responsive web design, people aren't setting that. They're saying with 100% or with whatever, and not setting heights. How does that impact accessibility?
