Insanity: You can't put a constraint on a table saying that, oh there can't be two records with this. This is where you could potentially use a materialized view or you could use a trigger. In order to do this, you actually need true serializable isolation. Most databases don't allow you to create constraints on multiple objects. SQL server, postgres, Oracle and MySQL will not prevent rights.
