Indexing - Is There Room to Squeeze Costs Lower?

CEO and chairman Bill McNabb said he still felt there was room to squeeze costs lower. Can indexing ever get too big or can indexing ever take over the market as some people have alluded? Or is there a place for a combination of indexing and active managers in the actual marketplace? "It's not in the nature of things that indexing could be 100% of the market"
