The Special Demonstration Squad

The Special Demonstration Squad was set up in 1968 to infiltrate activist groups. The officers would change their names and steal the names of babies that had died so they could create an identity. They were known for growing their hair long, looking the part and all the while sleeping with activists who didn't realise it was a police officer. But we have to assume that someone somewhere in the chain of command was perfectly aware of what was being carried out by these officers. And this inquiry doesn't answer that question yet.
