The administration must be held responsible for the inadequate planning for so-called phase four, the post-war stabilization. The U.S. did not have sufficient troops or sufficient plans to really deal with the dynamics of the situation that quickly emerged. In January of 2003, there were two intelligence analyses that broadly, in broad strokes, didn't predict that there would be the type of chaos that emerged but suggested that that was a possibility.
