CTA should be the initial test and the hemodynamically unstable patient. Discontinue aspirin for primary cardiovascular prophylaxis because of the risk of rebleed, probably continue aspirin for secondary prevention. Anti platelet, anti thrombotic and an anti coagulation medications are high points. No NSAIDs in patients who have had a lower GI bleed because it's associated with higher risk of re bleeding.
