The Military Orders of the Crusader States
The Hospitals predate the Crusader states. The Templars evolved about 20 years after the first crusade, initially as pilgrim escorts protecting people between the ports to Jerusalem. And then they came to take on the garrisoning of castles and the provision of troops. They're also extremely rich because of all those resources they're bringing forward every year.
