10-16cm of residual tibial bone should be left. This ensures an adequate lever arm, while a longer amputation than this gets into the Achilles tendon which has suboptimal blood supply and ability for soft tissue cushioning. The modified earl is a baloney amputation designed to enhance prosthetic end bearing. It requires longer operative internicate times compared to standard trans-tibial baloney amputations. About 8-12cm from the ground are needed to fit most modern high impact prostheses. With a long posterior flap, dog ears can be prevented with a well-designed incision line.
