How to Treat Humeral Shaft Fractures

As closed, humeral shaft fractures including low velocity gunshot wounds should initially be managed with a sling or splint. Interreduction internal fixation is absolutely indicated for open fractures and vascular injury requiring repair. The anterior or brachialis split approach to the humerus is used for middle third shaft fractures. Functional bracing extends from 2.5 cm distal to the axilla to 2.5cm proximal to the humeral condiles. A sling should not be used to allow for gravity-assisted fracture reduction.
