General Surgery
The practice of general surgery can change depending on whether you're inpatient or outpatient. In the urban setting where there's a lot more competition, you probably have to have a niche within surgery that is the focus of your practice. But for me, most of what I do is benign and malignant disease of the pancreas - billy-ary tract stomach. So I still fix hernias, take out gallbladders, lumps, bumps, and find that very rewarding.
